The City Clerk's office provides two formats for the Boards and Commissions Application form. You can download and print a hard copy to return to the City Clerk via USPS, fax, scanned email attachment, or in person at City Hall. You can also submit your request electronically.
Links to the application forms will be activated only during open recruitment. Applications submitted outside open recruitment periods may not be retained.
